1 Who We Are

NEXT Healthcare Solutions, LLC ("NEXT," "we," "us," or "our") is a physician-led healthcare management organization headquartered at 3535 Little Rd., Trinity, FL 34655. We own and operate Accountable Care Organizations (ACOs), a Medicare Advantage MSO, and Commercial Value-Based Care (VBC) contracts in partnership with independent primary care practices.

This Privacy Policy applies to the website located at www.nexthealthcaresolutions.com (the "Site") and describes how we collect, use, disclose, and protect information when you visit the Site or submit an inquiry through any of our forms.

2 Information We Collect

Information You Provide Directly

When you submit a contact form, an ACO enrollment inquiry, or schedule a consultation through our Site, we may collect:

  • Practice name and specialty

  • Practice state and number of providers
  • Current payment model participation (e.g., MSSP, fee-for-service)

HubSpot (CRM & Forms)

Contact and enrollment form submissions are processed and stored by HubSpot, Inc. HubSpot stores your submitted information in a CRM system used by our advisory team to manage follow-up. See HubSpot's privacy policy at legal.hubspot.com/privacy-policy.

Google Analytics & Google Tag Manager

We use Google Analytics (via Google Tag Manager) to understand Site usage. Google Analytics uses cookies to collect anonymized data about how visitors use the Site. You can opt out at tools.google.com/dlpage/gaoptout. See Google's privacy policy at policies.google.com/privacy.

Google Ads

We use Google Ads conversion tracking to measure the effectiveness of our advertising campaigns. This may place a cookie on your device when you click an ad. See Google's ad settings at adssettings.google.com.

Calendly

Our Site includes an embedded Calendly scheduling widget to allow you to book a consultation. If you use Calendly, your name, email, and appointment details are processed by Calendly, LLC. See Calendly's privacy policy at calendly.com/legal/privacy-notice.

We may also share your information with trusted service providers who assist us in operating our business, subject to confidentiality obligations. We do not share your personal information with unaffiliated third parties for their own marketing purposes.

5 HIPAA Notice

Important: This website and its forms are used exclusively for business inquiries and ACO enrollment interest. We do not collect, transmit, or store Protected Health Information (PHI) through this Site. Do not submit any patient health information through our website forms.

NEXT Healthcare Solutions operates as a healthcare management organization and, in the context of our ACO and VBC programs, we handle PHI only through separate, HIPAA-compliant agreements, systems, and business associate agreements (BAAs) with our member physician practices. Those activities are governed by our HIPAA Notice of Privacy Practices provided to covered entities directly.

This Privacy Policy governs the website only and does not describe how we handle PHI under our operational ACO agreements.

6 Cookies & Tracking

Our Site uses cookies — small text files stored on your device — to enhance your experience and to collect analytics data. The types of cookies we use include:

Automatically collected usage data (analytics) is retained per Google Analytics' standard data retention settings, which can be configured to 2, 14, 26, or 38 months.

8 Your Rights & Choices

Depending on your state of residence, you may have certain rights regarding your personal information, including:

  • Access — request a copy of the personal information we hold about you
  • Correction — request that we correct inaccurate information
  • Deletion — request that we delete your personal information

  • Do Not Sell — we do not sell personal information; no opt-out is required

9 Children's Privacy

Our Site is intended for healthcare professionals and is not directed to individuals under the age of 18. We do not knowingly collect personal information from minors. If you believe we have inadvertently collected information from a minor, please contact us immediately and we will delete it.

10 Changes to This Policy

We may update this Privacy Policy from time to time. When we do, we will revise the "Last Updated" date at the top of this page. For material changes, we may notify you via email (if you have provided it) or by placing a notice on the Site. We encourage you to review this Policy periodically.

Continued use of the Site after changes are posted constitutes your acceptance of the updated Policy.
