Who Can Enroll Through NEXT

The ACCESS Model is open to a wide range of primary care and referring organizations. NEXT supports enrollment for:

  • Independent primary care practices — solo physicians and small groups serving Medicare beneficiaries.
  • Multi-site primary care groups — physician-led organizations with multiple locations.
  • Federally Qualified Health Centers (FQHCs) and Rural Health Clinics.
  • Specialty practices participating as referring or partnering organizations under the model.
  • IPAs and clinically integrated networks coordinating enrollment for participating practices.

What is the CMS ACCESS Model?
The ACCESS Model is a CMS Innovation Center initiative designed to strengthen primary care for Medicare beneficiaries. It combines predictable payment, care coordination support, and aligned quality measures to help primary care practices deliver more proactive, beneficiary-centered care.
When does the program launch?
The ACCESS Model launches on July 5, 2026. To be active for launch, practices should complete enrollment by July 1, 2026. NEXT recommends starting the enrollment conversation 60 to 90 days in advance to allow time for application review, contracting, and operational onboarding.
Who can enroll in the ACCESS Model through NEXT?
Independent primary care practices, multi-site physician groups, FQHCs, Rural Health Clinics, specialty practices serving as referring partners, and IPAs / clinically integrated networks coordinating enrollment for member practices. If you're unsure whether your organization qualifies, a NEXT advisor can confirm in a 15-minute call.
What does NEXT do during enrollment?
NEXT handles the operational and administrative work of enrollment — eligibility review, CMS application, attestation, contracting, beneficiary attribution setup, and care-coordination workflow design. Once you go live, you receive a dedicated Clinical Care Coordinator, claims analytics dashboard, and ongoing CMS reporting support.
Will my practice remain independent?
Yes. NEXT is physician-led and exists specifically to keep independent practices independent. We provide the operational infrastructure of a large system — analytics, care coordination, CMS reporting — without consolidating ownership or control of your practice.
I'm a Medicare beneficiary — what does this mean for me?
If your primary care provider participates in the ACCESS Model through NEXT, you'll continue seeing the same physician with no change to your Medicare benefits. You may notice more proactive outreach — for example, reminders about preventive screenings, chronic-condition check-ins, and care coordination between visits — all designed to keep you healthier.
